It is reasonably stable, I used it with a nikon d500 + 70-200 f2.8 for low level mounted on an upside down Acratech long lens head which in turn was mounted on an Acratech Ultimate head. There are limits and this will depend on the tripod (I use my RRS TVC-33 for greater stability) and a sandbag counterweight. Do not overtighten the locking handle, mine broke easily (Fisol were great and sent a replacement).
