It is possible that the lever could be accidentally hooked on something and it could open without intending to, though it's never happened to me and I've hiked with tripod and camera over my shoulder a number of times. The screw knob would be the most positive and of the two if a person was really worried about accidental opening.What I like best about the lever clamp is the speed of opening, which facilitates quick swap of the camera from portrait to landscape orientation (if you have an L-plate) and fast adjustments of the camera's position in the clamp when adjusting centering.
