Reasons why it may not work could be older firmware on the camera, or maybe the eye is too small a part of the frame for the camera to pick it up. Depending on the body you're using, the camera might have a hard time picking out the eye if it's just a really small part of the frame. This happens to me sometimes on my a6000 when doing full body portraits. Make sure you're using the Wide or Zone for the Focus Area. I use AF-S because the a6000 doesn't support EyeAF when using continuous autofocus (AF-C). But newer bodies shouldn't have this issue.
