A: Yes it does. It comes with an L bracket and shoe mount. One nice thing about the bracket is that it can also be used to mount an umbrella.

A: Take the bracket to a hardware store to match up a new bolt for the missing one. All of the bracket thumb bolts have the same thread pattern, so you only need to get one long enough to do the job (around 5/8 .) You should also be able to get a press-on plastic cap to cover the bolt head for a cleaner job. I've been there. Easy fix - good luck!
