A: I mounted this leveler on a flat plate and could not move the locking lever. I had to use a fender washer between the leveler and the plate to make room. which, of course, allowed the leveler to spin a bit. I then had to tighten it down extremely well to get it to sit still.

That depends on your tripod's architecture. The HB-75 has a telescoping handle. At its shortest the whole unit is 4.25" long and about 2.5" are under the tripod. So your tripod will need to provide at least 2.5" of clearance to the ground for the HB-75 to work as intended.

A: This is unlikely. The bubble level is recessed slightly and would not get touched by the head. The thing you should be aware of is your bubble level will be covered. You may want to get a separate bubble level.
