A: It has a smooth back. No mounting holes. I don't think it was designed to mount to anything. I use it to power LED lights via battery that have 4 pin XLR power input but no battery mount. The cord is long enough to run the light up just over 6 feet leaving the battery resting on the ground.

A: According to the manufacturer, this adapter does have one D-tap port on it.
