A: The rule of thumb is that you increase the strength of the filter with the focal length. Depending on the desired look, I use 1/8 for wide angle lenses up till 28mm and then use 1/4 from 35mm-75mm and if needed I use 1/2 from 85mm and upwards to keep a fairly consistent effect on all focal lengths. Obviously it depends on your preference and how strong an effect you want.

A: Yes the dots on the filter is what softens the image and makes the highlights bloom. Those dots are a normal part of the filter.
